Fees: Assessment $82, 20 hour course $190, workbook $20, Total=$292 (cash or m/o)

Assessment must be taken and fees must be paid in full prior to the date the course begins.
You must attend the course at the location where you sign up for class.

DO NOT BRING CHILDREN TO CARE FOR WHILE YOU REGISTER OR
DURING CLASS HOURS.

VERY IMPORTANT: ABSOLUTELY NO LATE ADMISSION!

Please be present for each class 10 to 15 minutes early.
Late students to class will be locked out of class and have to repay class fees.

NOTE: Pre-registration and payment of $75 (cash or money order) is required prior to class date.
WE MUST HAVE YOUR DRIVERS LICENSE # and SOCIAL SECURITY # TO PROCESS PAPERWORK.

ABSOLUTELY NO LATE ADMISSION TO CLASS.
PLEASE BE SEATED IN THE CLASSROOM BY 8:45am.
FRONT ENTRANCE IS LOCKED AT 9am. LATE ARRIVERS CANNOT BE ADMITTED.
COURSE FEE IS FORFEITED IF YOU ARE LATE OR DO NOT SHOW FOR CLASS.

LUNCH BREAK 12-1pm . NOTE: Front entrance is again locked; no late admision!
NO ELECTRONICS (cell phones, ipods,blackberries &the like) ALLOWED IN CLASSROOM.
NO CAPS/HATS/DO RAGS/HOODS or SUNGLASSES CAN BE WORN IN CLASSROOM.
PLEASE DRESS APPROPRIATELY FOR THE CLASSROOM.

NOTE: Please advise at the time of registration if you need Level II, an 8-hour course. This may be recommended if you have an out-of-state license or had a traffic violation in another state. STudents needing an 8-hr class will be included in the classes marked by an asterisk (*).
